Player's Health Health Record System

Player's Health
It is part of the Appleton North Sports Medicine Team’s vision to strive to stay on the leading edge of knowledge, practices, communication, and record keeping while providing the most aggressive, yet safe approaches to athlete health care. In efforts to achieve this, Appleton North has integrated Player’s Health as its health record system.

Player's Health is a HIPAA-compliant, mobile, web-based athletic health record. Each athletic health record is created by, managed by, and accessible to the youth athlete’s caregiver, coach, and healthcare provider – including Appleton North’s Athletic Trainer.

Players Health is a mode of communication in which the healthcare provider (often times, the athletic trainer), coach, and parents of the student-athlete can correspond regarding injury diagnosis, prognosis, and treatment.

With the addition of Players Health at Appleton North, this will replace the paper copy of the medical consent/emergency card. As of November 2016, paper copies will no longer suffice. To create your child’s athletic health record, please go to www.playershealth.com. Click ‘Login’ at the top of the page, then enter your email and click ‘Sign Up’.

Please direct any questions/concerns regarding Players Health to Appleton North’s Athletic Trainer, Cassy Amo. Thank you for understanding and adhering to the changes set forth by the sports medicine team.
